01Duke in Forms
Say what the form is for and Duke proposes the questions, where each answer saves and what happens after. Nothing is made or published until you approve it.
Who decides
Duke drafts the questions and the after-sending steps, and makes the form once you say yes.
Duke reads the answers your role may see and tells you what came in.
New forms start from ideas made for your business type, such as a service request, an admission enquiry or a trial booking, or from a blank form.

Build
Add a text box, a longer answer, a dropdown, multiple choice, a date, a number, a file upload or a consent tick. Each answer says where it saves: the contact's name, phone or email, one of your extra fields, or the lead. A question can show only when an earlier answer matches.

After sending
When someone sends the form, the contact is found by phone, then email, or made new, so one family never becomes two. Choose whether it starts a lead in a pipeline stage, who is told, and whether visitors see a thank-you message or go to another page.
With Duke: a new answer lands on the contact's timeline, so it knows about it when you ask.
What happens
Share
Once it's published, a form gets its own link for WhatsApp, Instagram or SMS, a QR code and a printable A4 poster, a section for any page of your website, and a code to paste into WordPress, Wix or any other site. Give each poster or post its own link ending to see where answers came from.

04Questions
Yes. Copy the form's code and paste it into WordPress, Wix, Squarespace or any page's HTML. It shows in a frame with the height you set, and every answer lands in DukeOS.
Yes. Each answer finds the contact by phone, then email, or makes a new one. You choose whether it also starts a lead in one of your pipelines, and who on the team is told.
Forms have a hidden trap field, a minimum time to fill in and limits on how many answers one address can send. Real visitors never see a puzzle.
Not yet. A form collects details, files and consent. To take a payment, link to booking or your shop on your website, or send an invoice.
